Job Description

Working within Adult Social Care you will be required to respond to crisis referral's for adults who have found themselves in crisis and are in need of an assessment.

Have you previously worked completing paperwork following a stay in hospital, understanding what care package will suit and be able to understand the needs of each individual.

Working with an internal system you will be required to keep electronic records of every conversation and must be able to understand all of the care terminology.

This role is for a 35 hours a week over 5 days with flexible working.

Adult Social Care knowledge is essential along with an Enhanced DBS on the update service and able to provide two references covering two years one of which must be to demonstrate care experience.
