Our client, a reputable care provide in the area, is looking for an excellent, experienced Care Manager to join their friendly team. Our client offers a supportive living and learning environment for young adults who have severe and profound learning difficulties. Working with people with multiple disabilities, our client provides a transitional opportunity moving from education into adulthood with a focus on person-centred programmes to promote independence. As a Care Manager, you will support the Service Manager to effectively manage and coordinate all aspects of a high quality personalised service to young adults with complex needs and their families.

The Role:


As Care Manager you will ensure that individuals are fully involved in the development, coordination and delivery of all aspects of the service, in accordance with ways which are meaningful and realistic to each individual.

You will promote a culture of high standards, expectations and continuous improvement that values and empowers staff.

You will enable young people to achieve positive outcomes and to enable them to participate as full and active citizens in their local community, pursuing their hobbies and interests. This may include maintaining social networks and supporting the individual to access education, employment or social activity such as visits to clubs, pubs, restaurants, shops etc.

You will be responsible for risk assessments for individuals that affect their choices and lifestyle.


About You:


To apply for this role, you must have experience working with young people and adults who have complex needs.


You must have your Level 4 in Health and Social Care.


You must be a driver with access to your own vehicle.
