Job Description

Is your background in Mental Health, Learning Disability, Dementia, Cognitive behaviour or understand behaviours and how to manage them?


We are looking for Support Co-ordinators/Care team leaders in the Bristol area for well-established client in Bristol and Devon.


This role includes delivering support with everyday activities and managing a small team of support workers.


Our general aim is to support and encourage the people we care for to take control of their lives and live as independently as possible.


To improve the quality of life and enable people to live in their own homes, within their local communities and according to the individual's needs and preferences; the Support Coordinator is required to work in partnership with the people we support, the community and other members of their circle of support.



The ideal candidate will have experience working with a range of individuals in the health and social care sector that may include, people with learning disabilities, mental health, people with physical disabilities or people on the autistic spectrum.


The ideal candidate will also have experience in:

· Writing clear risk assessments.

· Support plans

· Attend Management meetings and be able to hold team meetings.

· Supervising staff

· Desirable: the candidate has a working knowledge of current legislation and best practice in health and social care.

· Be able to create an effective Rota.

· Cover on call mobile per rota


Candidate with NVQ Level 3 in health and social care or equivalent is desirable, you will need to be able to work flexible hours, including weekends and sleep-ins (£70), to meet the needs of the people we support.

Car drivers is essential

Someone with PBS or PBM training would be great but not essential

Ongoing learning and development support and opportunities are available.
