We are currently recruiting for two experienced and dedicated Home Care Coordinators to lead a team in delivering high quality, person centred, Domiciliary and Outreach Services throughout Lancashire.
Working for a fantastic organisation with over 16 years' experience in their field. They specialise in domiciliary care, providing professional and person-centred services and offer a wide range of high quality, person centred and outcomes focused Support services to support people with Autism, Asperger's, learning disabilities and those with profound and complex needs.
As the Care coordinator your main base will be the Office, based centrally in Preston, Lancashire you will your team to provide the best professional and person-centred care. Your other duties will include calls on the electronic rota system (Webroster) and ensure weekly rotas are sent out to staff in a timely manner, follow up the initial contact between the care worker and service user to evaluate the Care Plan and ongoing needs, to develop and maintain relationships with your service users and care staff team. You will establish this by undertaking regular telephone assessments which monitoring the service user's satisfaction and care worker performance. On call duties will be shared on a monthly basis.
The successful candidate will have relevant leadership and rota management experience, you will hold a Level 3 in Health and Social Care and hold a full UK Driving Licence.
In return to your hard work you will work Monday - Friday 9-5, receive a salary of between £18,500-£21,820, further training and development.
