Job Description

TEMP TO PERM OUTREACH SUPPORT WORKER POSITION (LEEDS; WEST YORKSHIRE)

Do you have paid experience of working with & supporting vulnerable young adults?

Have you previously worked within an outreach position?

Are you a car driver?

Are you currently on the DBS update service?

Are you immediately available?

If you can answer yes to all of the above questions we have the role for you.

Our client pride themselves on supporting young people leaving care; CSE; mental health; youth offending; drug & alcohol misuse & homelessness. Their goal is to provide a safe environment & settled accommodation for these young people along with a wide range of advice/support.

You will support the team manager by overseeing a cluster of supported tenancies and outreach within the Leeds area. Work across the region to ensure high standards are maintained in each property, ensuring quality and safe support.

Principal Accountabilities
"Support with the move-in and out process for service users, ensuring all paperwork is completed to a high standard and service users receive a warm welcome and a full induction.
"Reduce voids (empty beds) and support the growth of our client by planning effectively for move-on and liaising with Team Managers and placement teams regarding new referrals.
"Act as an enabler by assessing, in consultation with Team Managers and Support Workers, those service users who are ready to be transitioned to supported tenancies, ensuring that the appropriate support and benefits will be in place.
"Work collaboratively with regional teams and the wider business to improve and embed good practice models.
"Assist with the delivery of the ASDAN programme, improving the independence skills and educational achievement of service users.
"Ensure recordings are compliant with company policy and carried out in a timely manner and to a high standard
"Work across clusters to support other teams when necessary and share learning.
"Take an active role in multi-agency meetings, communicating professionally and effectively with social workers and other relevant professionals
"To effectively use IT packages to support the work of the company
"To undertake petty cash processes in line with company policy, ensuring teams budget effectively and complete correct logs.
"To work flexibly to fulfil the needs of the company.
"To work in a positive and pro-active manner, supporting staff and service users.
"Provide relevant and appropriate data for the purpose of monitoring and evaluation and produce reports to the Line Manager in line with commissioner and company requirements.

Package

  • This is a temp to perm position & you will be paid £10.18 per hour plus holiday pay. After successful completion of 12 weeks you will become offered a permanent contract subject to satisfactory references & placement within the 12 week.
  • Full company training & induction to the services
  • Working hours are 9am - 5pm over 7 days on a rotational basis dependant on the requirements of service users
