Job Description

I am currently working with with a client who has accommodated and supported over 7000 vulnerable people in a variety of local communities and they are looking for a confident new number to join their team.

Job Purpose:

To be responsible for the provision of Support for Vulnerable People (Service Users) for the Company.

Principal Accountabilities

1. To assist the Service Users allocated Social Worker in the delivery of the service.
a) Provide transport/escort to Service Users as directed in line with care plans
b) Conduct welcome and induction for Service Users
c) Ongoing assessments of Service Users' needs
d) Provide ongoing support and monitoring of Service Users
e) Assist in welfare and NHS issues
f) Preparation of reports
g) Arrange appointments with various partner agencies
h) Maintaining regular contact with Service Users
i) Informing Service Users of support groups
j) Assist Move-On of Service Users

Qualifications / Requirements-
High standard of general education.
NVQ Level 3 Health & Social Care - Children and young People (CCYP) or equivalent.
Enhanced DBS record.
Willingness to undertake further training.
Current full driving licence with business Insurance
Experience-
In a relative social care environment.
Experience of working in a Residential setting
In dealing with individuals from ethnic minority groups.
In dealing with vulnerable people from possible problematic background.
In conflict management.
Frequent use of computerised admin systems in busy working environment.

This position is part time at 20 hours per week and based in Morley LS27. The pay is at £10.37 per hour and there will be the opportunity for over time.
