An excellent opportunity has arisen for Service Coordinators based in the Preston area.

You will be working for an excellent company, specialising in high quality support to adults and young people with learning disabilities and complex needs.

As the Service Coordinator, you will be ensuring the correct support is provided to the individuals. You will implement, monitor and review support plans, risk assessments, follow guidelines and protocol. Also, liaising and working alongside health professionals and social workers effectively. As the Service Coordinator, you will be ensuring the correct support is provided to the individuals. The successful candidate will be a passionate team player with great communication skills, be confident and be able to lead by experience. You will also have the drive and commitment to work hard whilst having the correct attitude.
You must be flexible with your working hours as weekends, evenings and sleep ins are required.

You must have the experience of working with learning disabilities, mental health or residential setting experience with the Level 2 or 3 Health & Social Care qualification.

This is a full-time position with a salary of £19,675 with additional £46 per sleep in.
