We are currently looking for amazing support workers who are medication trained and have a minimum of 12 months up to date experience working within care.
Your role will be:
To support predominantly older adults, including those with physical disabilities, learning disabilities, long term health conditions, dementia and/or mental health conditions. To enable the service users to continue to lead active, independent and fulfilled lives whilst living in their own accommodation in an Extra Care scheme setting.To support adults, some with physical disabilities, learning disabilities, long term health conditions, dementia and/or mental health conditions to continue to lead active, independent and fulfilled lives whilst living in their own homes.
Facilitate and support service users in decision making; promoting choice and independence. To do this, you will:
* Support service users in all aspects of daily living including housing related support, delivering personal care, meal preparation, dressing etc.
* Provide support as identified in individual care plans and acting as key workers to allocated service users.
* Support service users in making claims for housing and other benefits and to support new service users with the completion of tenancy agreement forms.
* Assist new service users moving home, by ensuring that they receive the necessary support and advice to enable them to access services and amenities.
MUST HAVE A MEDICATION PRACTICAL IN DATE CERTIFICATE
Must have clear DBS and be on the update service or be willing to process one for £50 fee.
Must have 12 months up to date care experience.
No gaps in employment in the last two years longer than 1 month.
