Team Leader Job Description

To manage the day-to-day provision of high quality care and support for learning disabled service users within a housing and support service that fulfils the expectations of the organisation and funding and regulatory authorities; line managing and supervising staff and effectively fulfilling associated administrative responsibilities.

Main responsibilities:

" Plan and organise the delivery of support and care by your team, ensuring that there are up-to-date person centred support and care plans in place that are regularly reviewed; risk assessments are undertaken, incidents appropriately recorded, and there are adequate records in place to evidence the provision of support and care in accordance with contractual and regulatory expectations.

" Ensure clients receive support and care in accordance with their person centred support and care plan to foster their wellbeing and development and fulfil their potential for independence, choice, inclusion and control by, for example, supporting clients to:

o encourage and support the clients to maintain clean, tidy and welcoming living environment
o support clients to use appropriate laundry equipment so they can be enabled to do their laundry and clothing so they have suitable and clean clothing to wear

o plan, shop and prepare healthy and wholesome meals alongside the clients and taking into account their choices and wishes

o support clients to be healthy by assisting them to arrange and attend medical appointments

o support and encourage clients to maintain an active and supportive social life taking into account individual choices and wishes

o access training, work and support

" Ensure clients receive their housing related support by enabling clients to look after their finances (developing appropriate budgeting and financial training including support to access benefits and maintain them and assist them to understand and maintain their tenancy) and assisting clients to access services from the landlord including to access maintenance services and reporting nonfunctioning safety and security measures.

" Assist with or arrange for clients' personal care where required.

" Liaise professionally with clients' parents, social workers and others to make sure that the network of support around each client is functioning effectively.

" Attend and hold meetings related to the care function of the organization, reporting outcomes to the CEO and Registered Manager.

" Support, supervise and direct Support Workers and Co-workers to provide a high quality of support and care, making sure they complete their induction and this is recorded, have individual learning plans that are regularly reviewed and updated, are supervised regularly and supervision records kept, and conduct and record their annual review.

" Work collaboratively and co-ordinate the organisations' ethos, policies and procedures and actively promote the equality, diversity and the human rights of clients.

" To be available after hours to the staff and service users as required by the CEO and Registered Manager in line with the ongoing nature of the organisation's responsibilities to service users.

" Undertake such other tasks as required by the CEO and Registered Manager to help develop and maintain the high quality of service required.

" Undertake training and continuous professional development.

" Supervise the activity of any staff or contractors working in or around the properties (e.g. cleaners, maintenance men, gardeners, etc).

" Supervise any Guests or Local Volunteers ensuring they are able to make a meaningful contribution appropriate for their level of understanding and qualifications.
