Job Description

Job Title: Community Advice Hub Adviser - Mental Health
Location: Solihull
Salary: £23,600 - £24, 965

I am currently working with one of the UK's leading charities and community service providers. They have a fantastic opportunity to join their community team on a FTC as a community advisor. The role will be based out of Solihull and you will be providing advise and guidance to all that seek it.
Your day will consist;

To provide support to all residents of Solihull to ensure they are enabled to access all services they need to remain independent, safe, healthy and active members of their local communities.

To explore individual and family situations through the process of holistic assessment and provide the information, advice and support residents require.

To work to the Hubs ethos of prevention and early intervention but also support residents in crisis.

To undertake casework, completing benefit forms and information gathering.

To proactively promote the Community Advice Hubs to existing and new partner organisations, and to residents themselves.

Support the development of the reference tool of referral pathways, the services provided by partner agencies and local and national information tools, keeping the information updated on a regular basis.

To maintain databases and comply with monitoring and reporting requirements.

Communicate with and refer to partner agencies, working across the sectors and ensuring referral pathways are strong, relevant and appropriate, coordinating and monitoring referrals for those most in need and deemed most vulnerable.

Ensure that gaps in service and unresolved operational difficulties are reported to relevant Managers.

To work as part of the wider Community Advice Hub Team, take a flexible approach, and be led by demand across all parts of the service and wider organisation.

This role would be perfect for anyone that wants to work Monday to Friday 8 am to 4/5 pm in a central location. Someone who enjoys connecting with people and being a proactive problem solver. You will have fantastic communication skills both written and verbal. You will also have a good understanding of IT applications. If you've worked in mental health or have a passion for mental health that will also be ideal. Being local to Solihull is a plus.
