Brook Street Social Care is offering a new exciting and rewarding role as a "Floating Support Worker", across different services (Supported Housing, Outreach, Criminal Justice, Domestic Violence, Refugee) and different locations (Exeter, Dorset, Somerset, Basingstoke).
You will be providing high quality direct and person-centred support to vulnerable adults and/or individual with complex needs. To assess and support clients to develop independent living skills within a wide range of social issues.

WHAT YOU WILL BE DOING:

  • Build positive and effective working relationships and agreements with relevant external agencies.
  • Demonstrate good organization and planning skills with the openness to a changing work pattern/location at short notice.
  • Ensure that administration is done promptly and to a high standard using the electronic case management system; with detailed, concise, and accurate records.
  • Manage the planned and positive move-on of clients through the service, ensuring staff are supported to meet their targets around their length of stay.
  • Comply with and implement our Health and Safety policies, ensuring all checks take place and are recorded.


ABOUT YOU:

  • You have experience or interest to dip in and out a wide range of services.
  • Ability to work out of hours, evenings, and weekends on occasions depending on the needs of the project
  • You are PROACTIVE, PASSIONATE, DRIVEN and ORGANISED.
  • You fully embrace the motivation and passion to advocate for individuals who have experienced homelessness /social exclusion.
  • must be comfortable travelling to multiple locations.

MUST:

  • DRIVING LICENCE and happy to drive ( lot of travelling involved)
  • HOUSING ADMINISTRATION AND ABILITY TO PLAN
  • EXPERIENCE WITH COMPLEX NEEDS INDIVIDUALS AND CHALLENGING BEHAVIOUR
